Now that you have completed the forklift training class for Hantover, you must still submit your picture for your license, and have a performance evaluation to make sure you can perform the forklift skills necessary.  Use the form here to upload a head shot (selfie) photo and your license will be sent to the main offices. Once you have completed your evaluation, your trainer will send you your license.

It must be less than 1.5 Meg and a file type of .jpeg - jpg or png.
